Athlon Physical Therapy Health Sciences Scholarship Due Friday March 16, 2018

Prerequisite Criteria: 

1. Graduating Senior
2. 3.0 GPA (minimum)
3. Active or Past Community Service Participation/Coordination
4. 1 Letter of Recommendation (Teacher/Administrator/Volunteer Coordinator, etc…)
5. 500 word (minimum) essay.

A. Please attach a Resume’ with your work experiences, volunteer activity, awards etc… 

B. Essay questions: 

 Who/what factors influenced your decision to enter the healthcare field? What makes you an excellent candidate to be a healthcare professional and why do you want to enter the field?

 What are your long term goals? Where do you see yourself in 10 years?

 What makes you a competitive candidate for this scholarship? Please tell us about your academic achievements, community service activities, examples of leadership roles, or other personal characteristics/experiences that make you deserving of this scholarship. (Please be sure to include why these things make you a good candidate, or set you apart from others).
